Booking Appointments

Here are few points to remember when booking your first appointment at one of our clinics:

  • Please inform our staff about any immediate dental concerns you are experiencing when looking to schedule an appointment.

  • When you come in for your scheduled appointment, either arrive a few minutes early to fill out a New Patient Form, or bring a completed form with you, which can be downloaded and printed at the bottom of this page.

What We Need to Know?

At your first visit, please have the following infomation available:       

  • Insurance cards or information

  • Names of medications you are taking, if any

  • If you are under 18 years of age, please inform us of your immediate family member (mom/dad, brother/sister) or a guardian

  • If you have visited another dental office prior to your appointment

  • If you have any major medical conditions or undergone major surgeries

  • Your medical doctor information
